Job summary

Are you interested in enabling patients to gain independence? Do you have sound communication skills and a thorough problem solving approach?

This is an exciting opportunity to work in the inpatient Snowdon ward, a 14 bedded Neurological Rehabilitation ward, providing specialistic patient-centred rehabilitation as part of a large multi-disciplinary team.

In the role you would carry your own and a joint caseload of patients with supervision from registered Therapists including Speech and Language therapy, Occupational therapy, Physiotherapy and Psychology, whom you would work with to identify and support to achieve rehabilitation goals.

Main duties of the job

You would have support and training to enable you to carry out:

*Assessment and treatment planning to include mobility and functional assessments, including the provision of basic off-the-shelf equipment and walking aids.

*Development and implementation of exercise and activity programmes in order to progress a patient's rehabilitation.

*Planning discharges home including environmental visits with or without patient to place of discharge to assess environments and, with support, to make arrangements for safe discharge home and to support transfer of care between teams.

*To participate in the ward keyworker role, which involves arrangement of patient planning meetings, liaison with key people i.e. team members, the patient and their NOK. You will be required to organise, signpost and delegate tasks within this role.

Qualifications

Essential

  • No formal qualifications are required, but good literacy and numeracy and IT skills are essential
  • NVQ 4 (or equivalent experience) in care
  • Hold a valid full driving licence which enables them to drive in the UK and insurance for business use

Desirable

  • Foundation Degree in Health and Social Care or working towards it.

Experience

Essential

  • Experience of working with people with a cognitive impairment who have complex needs and may present challenging behaviour.
  • An ability to work according to a person-centre approach based on empathy, understanding and respect is essential.
  • Experience of working with a multi-disciplinary team approach.
  • Experience to motivate and deliver rehabilitation to patients for whom initiation can be challenging.

Desirable

  • An understanding of non-aversive approaches to care is essential.
