Job summary
HIOW Healthcare is looking for inspirational RN's to deliver a high standard of quality care to patients within the community setting. Community Nurses work autonomously & as part of a team and care is delivered in the patient's own home environment (including residential care) & designated treatment rooms. Nurses are responsible for a manageable caseload, working across the city ensuring our patients are receiving the best quality care from their own homes & thereby avoiding hospital admission.
HIOWH Community Nurses work with adults who need support for a multitude of health conditions. These include:
Long term conditions management
Acute wounds & complex care
Palliative & EOL care
Facilitating discharge from hospital, drain management, administration of prophylactic medication
Continence care including catheter & bowel care
PICC line management
Administration of a variety of medications
As a lone worker, you will be supported by an experienced & nurturing team who are always available, to allow you to develop your existing skills into becoming an autonomous practitioner in the community. You will be provided the training you require to maintain & develop your clinical skills.
You will work within a team that is based at Bitterne Health Centre in Southampton. Our working patterns are across a 7 day week. Core shifts are 0830-1630 and 1400-2200, but we are open to discussing this. There is also a requirement that you will work 1 in 4 weekends as well as bank holidays.
Main duties of the job
*A registered nurse working as part of a multi-disciplinary Locality Health Team affiliated to a cluster of GP Practices providing nursing care to the Practice population both in the patient's home or clinic setting based on individual care needs.
*Delivering on the organisational objectives to meet individual health care needs through high quality, safe and effective services to avoid unnecessary admissions to secondary care, support early discharge from hospital, effective case management for patients with complex long-term conditions or following an acute health episode and individualised care for the patient and carers at the end of life.
*The post-holder will be required to demonstrate a high degree of autonomy and professional accountability, working within the Nursing and Midwifery guidelines to have responsibility for the management of a defined Community Nursing caseload within one or more GP practices in the absence of the Community Sister or Clinical Manager.
