Job summary

This is an exciting new opportunity to join our team at The Orchards, which comprises of a 20 bedded Acute Ward and 10 bedded PICU for both male and female adults.

We have been working with other local NHS Trusts to create a new organisation, bringing together all community, mental health and learning disability services from across Hampshire and Isle of Wight. Hampshire and Isle of Wight Healthcare NHS Foundation Trust has brought the services into a single organisation which will result in more consistent care, more equitable access to services irrespective of postcode, and a more sustainable workforce and services. The new organisation will operate locally to ensure services can best meet the needs of different communities.

The Carer's Support Lead works collaboratively and aims to assist in improving the wellbeing of families and carers of people who are admitted to an Adult Inpatient Mental Health Unit by supporting carers directly, either via 1:1 or groups, developing and maintaining relationships with the service provider teams, providing carers with updates of their relatives care and treatment, contributing to service development and research and representing carers at leadership meetings.

They will work to create independence, choice and recovery of a life beyond mental illness in a safe and timely way, in partnership and collaboration with people who use our services and their families.

Main duties of the job

To initiate, establish and maintain relationships with families and carers in a variety of settings, while developing trust and rapport.

Act as a coach and mentor to families and carers by providing information about support and services available for them, ensuring they have carers communication plans where required.

Work collaboratively with inpatient/CRHT/CMHT to support carers via 1:1 support or group settings.

To work within The Triangle of Care standards and undertake self assessments and action plans.
