Job summary

A new opportunity for a band 5 Occupational Therapist or Physiotherapist who is passionate about Frailty to join our expanding therapy team within Urgent Community Response (UCR) Service. UCR is a rapid assessment, diagnostic and treatment service, which aims to reduce admissions for people with complex health needs and frailty. As a Therapist, you would provide holistic assessments during the acute phase of illness. You would provide support to the team using your skills to resolve complex manual handling issues, and assess mobility and equipment needs. Expectations depend on your professional skill set. You will use a collaborative approach to prioritise crisis management and rapid assessment to prevent hospital admission and decrease the risk of further deconditioning during their illness. You will support patients' safety at home, aiming to support them back to their functional baseline quickly as they recover medically.

The UCR team operates from a hub in Herriard, located in-between Basingstoke and Alton. The service operates 0800 to 2000 7 days a week, 365 days a year. The successful candidate would be expected to work 1 weekend in 4. Hours to be worked within our operational times can be discussed during the interview process. This role allows you to work autonomously and implement a breadth of interventions, while having the support of a wider multidisciplinary team, including Consultant Practitioners, Advanced Clinical Practitioners, AHPs and Nurses.

Main duties of the job

To provide functional assessment and devise effective holistic therapy plans and goals for patients who are acutely unwell to enable them to remain as independent as possible in their own homes.

Assess, recommend, and organise the provision of adaptive equipment and minor adaptations within patients' homes.

Offer clinical supervision to support other team members such as HCSW's in their role.

To recognise a deteriorating patient and escalate to the MDT appropriately. The team with support this with training as required.

To work in conjunction with a wide range of clinical colleagues; primary care, community teams and social care professionals, facilitating a patient focused, co-ordinated case management approach across primary and secondary care for people who are most vulnerable to, and at high risk of repeat admissions to hospital.

Qualifications

Essential

  • HCPC registered Occupational Therapist or Physiotherapist
  • OR
  • Student apprentice within Occupational Therapy or Physiotherapy.

Experience

Essential

  • Experience working as an Occupational Therapist or Physiotherapist or as a student or apprentice in these professions.
  • Able to demonstrate a good command of the English Language both written and verbal.
  • Computer Literate in both data entry and communicating via IT - emails and reports
  • Demonstrates organisational skills, including the ability to make decisions, prioritise and plan workload according to patients' needs.
  • Demonstrate flexible working within a rapidly changing caseload.
  • To demonstrate the required behaviour in keeping with the Trust values
  • Demonstrate clear communication skills with the ability to communicate complex, sensitive or confidential information in an appropriate manner.
