Integrated Community Team- Petersfield and Bordon
Southern Health Foundation Trust has an exciting opportunity for experienced qualified Nurse to join the Petersfield, Integrated Care Team. Working in the community you will be providing care to patients' with complex care needs, enabling patients to receive the care they require to remain in their own home.

We are looking for an enthusiastic, caring & motivated professionals with excellent clinical, organisational & communication skills, who have the ability to work autonomously.
The post holders must have relevant experience in health care and demonstrate a proactive and positive attitude to learning the skills required to fulfill the role.
Access to a vehicle and possession of a full driving license is essential.

Main duties of the job
*To contribute to the provision and monitoring of high quality individualised nursing care in accordance with agreed policies and procedures
*Work independently under the leadership of the Registered Nurse to, plan, organise and deliver clinical care in line with service and patient needs.
*Nursing Associates are equipped with the appropriate level of knowledge, understanding, skills, attitudes and behaviours and will work within their scope of practice in accordance with the Nursing & Midwifery Council (NMC) code of conduct and standards of proficiency for Nursing Associates.

The final combination of Southern Health and Solent NHS Trust services is expected in late 2024, when our new organisation, to be known as Hampshire and Isle of Wight Healthcare NHS Foundation Trust, will be formed.
Our new Trust will continue to cater to the unique needs of different communities, making healthcare across the county more accessible, as well as offering staff more opportunities for career development, training and partnership working.
