Job summary

We are looking for an enthusiastic Community mental health nurse to join our friendly multi-disciplinary team as a Care Co-ordinator.

We are able to offer an individualised program of support and professional development to work through with the encouragement from an existing motivated, positive and experienced team. Newly qualified professionals are encouraged to apply.

Main duties of the job

You will hold a caseload and undertake the role of care coordinator for those with varied complex mental health and social care needs. You will provide assessment and specific interventions to meet the needs of service users, their carers, and families. It is expected that you will be a flexible practitioner who enjoys new challenges in meeting the developing needs of the service and our service users.

The post holder will be expected to work proactively in forging professional working relationships with multi-agencies.

Car driver is preferrable but not essential. You will be working 9am -5pm Monday to Friday and occasional weekend or bank holidays. We are flexible regarding working patterns and working hours.

The final combination of Southern Health and Solent NHS Trust services is expected in late 2024, when our new organisation, to be known as Hampshire and Isle of Wight Healthcare NHS Foundation Trust, will be formed.

Our new Trust will continue to cater to the unique needs of different communities, making healthcare across the county more accessible, as well as offering staff more opportunities for career development, training and partnership working.

  • Registered Nurse, Social Worker, Mental Health Practitioner or O.T.
  • Computer skills
  • Significant post qualifying experience
  • Mentorship course
  • Clinical supervision training
