Southern Health Foundation Trust has an exciting opportunity for a Clinical Pharmacist to join Shakespeare Road Medical Practice, a forward thinking and innovative GP Practice providing primary care services for 14,000 patients in Basingstoke.
We are looking for an enthusiastic, caring and motivated professional with excellent clinical, organisational & communication skills.
The successful postholder will provide a specialist resource as a Clinical Pharmacist Practitioner for our patients working with clinicians, nursing team and in collaboration with other members of the practice multidisciplinary teams. You will work within our practice community, as an autonomous, practitioner providing holistic care whilst assessing, diagnosing and providing treatment to patients. You will also assess, diagnose, treat, refer or direct to other services; You will promote self-care and educate service users to allow them to be empowered to make informed choices about treatment.

Main duties of the job
The Advanced Specialist Interest Pharmacist will act as the expert pharmacist for the given specialist area, they will draw upon extensive post qualification experience working with patients and the multidisciplinary healthcare team in primary and secondary care where relevant and with community care providers. They will be suitably qualified as an independent prescriber and will have in depth knowledge of medicines and applied therapeutics.
