Job summary
An opportunity has arisen for a Band 3 HCSW to join an established Heart Failure service. Based at Havant Health Centre the service covers Fareham, Gosport, Havant, Waterlooville, Hayling, Petersfield and Bordon areas. Experience of phlebotomy, taking ECGs, Blood pressure, and knowledge of the trust IT systems would be desirable but not essential as training will be given.
The successful candidate will work with some autonomy but primarily will be supporting the nurses to deliver care to patients with Heart Failure. You would be expected to see patients in their homes or in a clinic setting, liaising with their families and health care professionals in a professional manner.

Main duties of the job
The Health Care Support Worker will have responsibility for participating in the delivery of patient care under the supervision of the Community Heart Failure Team. Following prescribed care plans and treatment protocols. The post holder will also provide general administration support to the team.
Work under supervision following written care plans and care pathways devised by the Registered Nurse.
Follow SHFT Policies and Procedures.
Has knowledge and skills to carry out prescribed tasks and will report to Registered Heart Failure Nurse any variances.
