The A31 Alton Primary Care Network has an exciting opportunity for you to join our team

We are looking for a Registered Nurse to join our dedicated and highly professional team of Nurses working across the Alton and surrounding areas. The Alton Primary Care Network serves three of the registered General Practices and you will play a pivotal role in keeping hospital admissions and re-admissions to a minimum as well as encompassing the dimensions of health service planning, advocacy and care planning for patients with a wide range of service needs.

Within this role, you will act as part of an ever-expanding multidisciplinary team, developing collaborative, needs based care plans, through assessment, planning, implementation and evaluation.

You will be required to provide important educational and advisory service for patients, offering information on various aspects of health care to help support patients living with their condition. Within this role, you will be expected to provide nursing intervention to patients which include; wound management, catheter care, medicines management as well as palliative care.

You will need to be effective in communication skills and must be prepared to work flexibly and collaboratively to meet the needs of the service.

We also offer our staff bespoke training packages, supportive team working environment, leadership pathways and comprehensive preceptorship programme for those that are newly qualified.

This post is open to both full and part time applicants.

Main duties of the job

Community nurses operate with a high degree of autonomy. As a result, community nurses value the variety their role offers and enjoy the professional relationship built with colleagues as well as patients. Community nurses deliver bespoke care and working closely with patients and relatives, to ensure care the care that is delivered is person centred and tailored to meet the patients needs and wellbeing.

Community nurses play a pivotal role in delivering persons centred care to patient using their expertise and clinical judgement to make informed decisions to improve the care quality of the patient.

Main tasks include venepuncture, medicines management, catheter care, palliative and end of life care as well as both acute and chronic wound management.

The final combination of Southern Health and Solent Trusts is anticipated to occur in July 2024, when the new organisation will be formed.

Our new Trust will continue to cater to the unique needs of different communities, making healthcare across the county more accessible, as well as offering staff more opportunities for career development, training and partnership working.

  • Experience of multi-professional working
  • Car driver and own transport
