Job summary
An exciting opportunity for a Band 5 Physiotherapist to join our Integrated Community Team. You will work closely alongside our nursing and frailty teams and wider Primary Care Colleagues to prevent hospital admissions, support patients being discharged from hospital, palliative care and promote self-management for patients with long term conditions.
We are also part of the delivery of UCR (Urgent community response) model of care whilst continuing to deliver rehabilitation to adults living in the area who have long-term or complex health needs.
We are looking for applicants who are dynamic, keen to develop their skills and who enjoy working as part of a team. The successful candidate will need to have good communication skills, demonstrate good time management and prioritisation skills. The post holder will participate in the development, training and support of the multi professional team and students. Applicants must be a registered physiotherapist and have full access to a vehicle for work and possession of a full driving license is essential. The post holder will also need to be willing to work flexibly to support our 7 day service model.

Main duties of the job
Working within Professional guidelines and accountability, the post-holder will have responsibility for the management of a defined community caseload (via Nursing, mental health, Physiotherapy or Occupational Therapy interventions) for a group of GP Practices as part of the Integrated Care Team.
The post holder will have responsibility for managing, co-ordinating and delegating work ensuring referrals are dealt with according to clinical need, holistic assessments and care plans are recorded in line with Trust and Professional record keeping standards and clinical care at all times meets acceptable standards of patient safety and quality outcomes.
The post holder will be required to work with a high degree of autonomy and professional accountability. A primary aim of the role is to prevent unnecessary admission of patients to an acute hospital and support timely discharge into the community. The post holder will also have a responsibility for re-enablement, rehabilitation, health protection and promotion programmes that improve and reduce
