Job summary
To provide physical and psychological care to service users in accordance with the specified plan of clinical care and in line with the recovery model to service users who live in any part of the catchment area. To carry out duties as designated by the team leader. To provide support to the person responsible for the clinical care of the service user. To accurately record service user information. To provide support to relatives/carers. To provide support to nurses in the depot/Clozapine Clinic (including phlebotomy).
Main duties of the job
To record all contacts with service users using the electronic record-keeping system, To ensure effective communication with service users, relatives and carers, visitors, staff and others, To ensure that all communication is within the boundaries of service user confidentiality and to seek guidance when unsure, To exchange factual information with service users using persuasion, tact, reassurance, empathy and acknowledging barriers to understanding. To document information to support mental state examination. To ensure that any information related to service users or other personal safety is communicated immediately to senior team members.

The final combination of Southern Health and Solent Trusts is anticipated to occur in July 2024, when the new organisation will be formed.
Our new Trust will continue to cater to the unique needs of different communities, making healthcare across the county more accessible, as well as offering staff more opportunities for career development, training and partnership working.
