Job summary
We are recruiting for a Senior Community Support Worker to join our team.
The successful candidate will support people with a learning disability and complex health, behavioural, emotional or mental health needs.
You will be responsible for assessing care needs and planning and implementing care. You will monitor and record complex behaviours and reporting back to the Community Learning Disability Team. Working with clients with complex behavioural needs and adjusting any relevant documentation.
Main duties of the job
To assist the Clinical Psychology team in the following areas:
*To provide support to service users and promote their rights, responsibilities and independence.
*To work as part of a team that provides health services to people with a learning disability.
*Support, directly and flexibly the specific needs of service users in accordance with their care plan.
*To work in partnership with the individual across boundaries of care, organisation and role, coordinated through a personalised care plan and Care Programme Approach.
*Promote opportunities that :

Promotes anti-discriminatory practices
Maintains confidentiality
Promotes equal opportunities and diversity
Ensures service users are treated with dignity and respect as part of ethical practice.

The transformation will happen in staggered stages; Hampshire CAMHS, part of Sussex Partnership NHS Foundation Trust joined Southern Health in February 2024, with Isle of Wight NHS Trust mental health and community services transitioning in early May 2024. The final combination of Southern Health and Solent Trusts is anticipated to occur in July 2024, when the new organisation will be formed.
Our new Trust will continue to cater to the unique needs of different communities, making healthcare across the county more accessible, as well as offering staff more opportunities for career development, training and partnership working.
