Job summary
A unique opportunity has arisen for a Specialist Community Forensic Team Practitioner for full time or part time practitioners. This role is an integral part of our established multi-disciplinary teams which include Doctors, Nurses, Psychologists, Occupational Therapists and Speech and Language Therapists.
We are looking for a Nurse, OT or Social worker to work within the team as a practitioner. Your role will be supporting discharges from secure services into the community. There is an opportunity for the candidate to offer specialist support within the area of substance misuse, this is not a requirement however desired.
The Specialist Community Forensic Service operates across Hampshire and the Isle of Wight and aims to reduce patients' length of stay in forensic units. The team is multi-disciplinary (including forensic psychiatry, mental health nursing, clinical psychology, social work and occupational therapy).
The team works with patients who have a diagnosis of mental illness and have been in contact with the criminal justice system and are transitioning out of secure inpatient services. The team work closely with our patients as they move into the community so that they can eventually live safely and independently with the support of our community mental health services. The focus is on offering the least restrictive care possible and providing an alternative to a secure inpatient setting.
Main duties of the job
*To act as care co-ordinator to an agreed caseload of service users with severe and enduring mental illness with a forensic history.
*To undertake ongoing assessment, treatment and evaluation for service users known to the service.
*To use professional knowledge to assess, monitor and review social care needs of individuals, relatives and carers within a specific care group in accordance with agreed competencies, quality standards and professional good practice in relation to case and risk management.
*To work in partnership with patients, carers, relatives and other agencies to identify current and future service requirements and contribute to service development within the scope of the role.
*As a core member of the MDT, facilitate safe and appropriate discharge for patients by co-ordinating the discharge process.
*Carry a case load of complex patients.
*Provide advice and guidance to staff within the service and refer to external partners when required.

The final combination of Southern Health and Solent Trusts is anticipated to occur in July 2024, when the new organisation will be formed.
