Job summary

We are excited to offer this new opportunity in a partnership role of Mental Health and Housing Practitioner. This role will cover the Winchester area and work with our colleagues in both mental health at Southern Health and the housing teams at the City Council. You will be employed by Southern Health and seconded into the City Council on a long-term basis (currently until March 2025).

The focus of this role is to predominantly work with those people experiencing homelessness, living in temporary accommodation, or those who are at risk of homelessness. Supporting the mental health of this often-complex section of the local community is key to enabling them to positively move on with their lives.

The successful candidate will need to be highly motivated, enthusiastic with excellent communication and organisational skills. They will need to be forward thinking, able to engage service users, families, staff and work in partnership with system partners and wider stakeholders in developing services.

Main duties of the job

To work in a clinical capacity providing mental health interventions including assessments and treatment to those clients at risk of experiencing homelessness or with a history of experiencing homelessness, as identified by Winchester City Council's Housing Options Team, or Southern Health's community mental health or the inpatient flow teams outside of established pathways.

To contribute to breaking down the barriers to accessing and sustaining health care and suitable settled accommodation for the identified client group. To actively work with other agencies to provide wider support and address underlying causes of homelessness, developing referral routes between the Council and partner organisations across the wider system (including statutory and voluntary sector services).
