Job summary
We are seeking a motivated Trainee Advanced Clinical Practitioner (tACP), Nurse or AHP to join our Urgent Community Response (UCR) and Frailty Virtual Ward (VW) team delivering care to patients closer to home and preventing avoidable hospital admission across North Hampshire. The ACP will deliver specialist practice in UCR working closely alongside the Consultant Practitioner, Senior Clinical Practitioner, Advanced Clinical Practitioners, and other Trainees supporting the delivery of care in the home settings of our population. The tACP will develop strong working relationships with the wider systems teams which may include community geriatrician, specialist doctor, GPs, and other MDT colleagues.
UCR focuses on preventing avoidable hospital admissions for patients with acute ill health conditions by providing advanced clinical assessments that place the person at the core, within 2 hours of referral. This includes implementation of treatment and appropriate monitoring whilst in crisis. The frailty virtual ward pathway supports patients to receive ongoing care that they may receive in hospital and prevents them from deterioration that would require an admission.
This is a trainee role with progression from band 7-8a
The service operates 0800-2000 7 days a week, 365 days a year.
We are happy to discuss flexible working but would expect as a minimum working one weekend in four.
Main duties of the job
The post holder will be a Nurse or AHP trainee, working under supervision, to develop a high degree of autonomy and complex decision making. This will become underpinned by a master's level award, completed during the training programme, that encompasses the four pillars of clinical practice, leadership and management, education, and research with demonstration of core capabilities and area specific clinical competence.
Please note that study dates will be reliant on the University timetable and relevant spaces.
They will manage, under supervision, clinical care in partnership with individuals, families, and carers, which will include the analysis and synthesis of complex problems across a range of settings, enabling innovative solutions to enhance people's experience and improve outcomes.
They will work towards exercising autonomy and decision making in a context of complexity, uncertainty, and varying levels of risk, holding accountability for decisions made. They will work alongside, leading and clinically supervising the registered and unregistered staff members within the team.
Develop values-based leadership skills within their team and developing across the care pathway, services, and systems in complex and changing situations. Contribute to a knowledge-rich and inquiry culture across the service and system that contributes to research outputs and has a positive effect on development, quality, innovation, increasing capacity and capability, and making systems more effective.
