Specialty Doctor in Elderly Care Medicine

Job summary

Southern Health NHS Foundation Trust is looking for a doctor with experience in older people's rehabilitation medicine to become part of the team at Petersfield Community Hospital (PCH).

The hospital has two rehabilitation wards, two OPMH wards, a community hub housing the community nursing and therapy teams and the community assessment unit (CAU). The CAU extends the advice, assessment and support we are able to offer to local patients both in the unit and into the community. Integral to this is the delivery of Comprehensive Geriatric Assessment for older people with frailty when needed, including the ability to step up into the community rehab wards.

Over the last 2 years SHFT has also developed Urgent Care Response services and UCR Hubs and last year established frailty virtual wards. Both of these services utilise the ability to step up patients into the rehabilitation wards and can support early discharges, where appropriate.

A single clinical IT system (RiO) is used across all of these settings supporting continuity of communication across our patient's journey.

Main duties of the job

The post holder's main responsibilities will be to work as a ward doctor as part of the PCH MDT team overseen by a Consultant Physician Geriatrician, a consultant nurse (advertised), with two advanced clinical practitioners, a second specialty doctor, AHPs and experienced ward nursing teams. The post holder will be based on the two rehabilitation/physical health wards; Rowan and Cedar.
