Health Care Support Worker

Community Assessment Unit - Petersfield Hospital

Band 3 23,177 - 24,335 Pro Rata

Hours: Part time 22.5hrs hours per week

Are you motivated and enthusiastic? If yes, an exciting opportunity has arisen for you to join a developing community frailty service working towards better local and integrated care for patients with complex medical needs requiring assessment from a Consultant Geriatrician, GP with Specialist Interest, Advanced Clinical Practitioners, specialist nurses and other allied health professionals.

We are looking for a Band 3 Health Care Support Worker to work part time. CAU clinics run between 0800-1600 during the week (Monday to Friday) and we are seeking a support worker to work three days to support clinics with the possibility of also supporting the Urgent Care Response service if appropriate.

The clinics are based at Petersfield Community Hospital. Any Urgent Care Response work in the community takes place between Bordon and Hayling Island and although not a prerequisite for the role, there will be opportunities to contribute to UCR if this if appropriate.

Main duties of the job

To work responsibly in carrying out patient care and assessment according to their individualised needs. To report to a qualified member of staff any observations/ concerns regarding individual patients. Help to prevent hospital admission and work towards better local care under the supervision of a qualified nurse. Assist in the day to day running of the clinics and office administration duties.
