Basepoint Business Centre in Havant and/or Basepoint Business Centre in Gosport.

As part of the role, the candidate will be provided with training and a comprehensive induction programme to help them meet required competencies. At Southern Health we are keen to develop our staffs' career in healthcare and provide support for further training and qualifications.

The role is with the Crisis and Home Treatment Team with the option to be based in Havant or Gosport team working across the division. The successful candidate will work in a multi-disciplinary team alongside Nurses, Social Worker's, Occupational Therapists, Clinicians and Admin staff.

The Crisis team provides intensive treatment plans to support patients in the community, averting hospital admission where possible, whilst also supporting individuals in their journey from the hospital environment to home.

We welcome applications for part-time set shift patterns an example could be, two evenings 4pm -- 10pm and/or weekends, with potential flexibility on start times. The work pattern would be agreed with successful candidates at the point of offer.

Owning or having access to a car is essential for visits to patients, the area covered includes Gosport, Fareham, Petersfield, Waterlooville, Hayling Island and Emsworth.

Main duties of the job

To work as part of a multi-disciplinary team, which supports individuals who are experiencing Acute Mental Health crisis and who require intensive input in order to remain in the community, averting hospital admission where possible. Whilst also supporting individuals along their journey through the Acute Care Pathway from the hospital environment to home. This role is key to supporting the Crisis Team practitioners, and will entail a shift-based working pattern that will help to cover a 24-hour service. The Crisis Team deliver intensive, recovery focussed interventions for individuals presenting with acute clinical needs as an alternative to hospital admission

You will support service users to identify their needs and to assist practitioners to plan, implement, review and evaluate individual care plans.

To work in a recovery focussed manner having the individual service user's needs at the fore at all times, supporting service users to work towards their identified goals.

To use agreed values and skills to underpin their day-to-day work.
