Job summary
Southern Health Foundation Trust has an exciting opportunity for a full-time Social Prescriber to join Shakespeare Road Primary Care Network.
We provide primary care services for over 13,000 patients in the Popley area of Basingstoke and the practice is in collaboration with Hampshire Hospitals NHS Foundation Trust, Solent NHS Trust and North Hampshire Urgent Care.
We are looking for an enthusiastic, caring and motivated professional with excellent organisational & communication skills.
The Social Prescriber works alongside a team within general practice and within the Primary Care Network (PCN) and empowers people to take control of their health and well-being.
Social prescribing can help to strengthen community resilience and personal resilience whilst reducing health inequalities by addressing the wider determinants of health such as debt, poor housing and physical inactivity by increasing people's active involvement with their local communities.
This role can be particularly beneficial to patients with long-term conditions, those with mental health issues and those who are lonely or isolated or who have complex social needs which affect their wellbeing.
Main duties of the job
Health Connectors provide a service which helps members of the community access support to better self-manage their health and wellbeing and long-term health conditions.
The project will initially work with individuals and the Complex Care Team following discharge from hospital, to support them to better self-manage health and wellbeing or long-term health conditions.
The Health Connector will be a member of the Health Connectors service which works across the locality. Health Connectors will work with individuals and groups utilising untapped community resources, support the facilitation of self- help groups and other innovative approaches to enable people to better manage their own health and wellbeing.
