Job summary

We have an opportunity for an enthusiastic and skilled mental health practitioner from a background in Mental Health Nursing, Social Work or Occupational Therapy to join us in the Community Mental Health Team. The team are supportive, optimistic, friendly and value collaborative working drawing on the diverse experience within the existing team.

The role of the care coordinator is to autonomously work with an allocated number of patients to support their recovery journey. This may include the use of psychological interventions, medication support, access to group work and carers support. You will need robust assessment skills to enable the team to deliver high quality care and treatment plans. Excellent communication skills are also essential to enable you to be a role model for others, as well as positively representing the team and the organisation as a whole. You must also be able to liaise effectively with other services, both internal and external to Southern Health Foundation NHS Trust, retaining service user focus at the heart of any clinical decision making to ensure the best outcome.

You will have the chance to develop assessment and risk management skills as well as your leadership skills. ily flexible working where possible and the role involves a mixture of being based in our Totton office, working from home and working out in the community.

*To act as care co-ordinator to an agreed caseload of service users with severe and enduring mental illness providing recovery based community mental health services.

*To work in collaboration with service users as experts in their own care, promoting the principles of recovery, choice and personalisation, which value and empower the service user

*To use tools which promote user empowerment e.g., Wellness Recovery Action Plans (WRAP), Recovery Star.

*To be proactive in ensuring service users receive the range of interventions they need in line with their care plan, whether or not the post holder provides those interventions. This will include physical health monitoring.

*To have a proactive approach to the role and needs of carers / families.

*To be proactive in monitoring mental health needs and respond in a timely and appropriate fashion if needs change adjusting the care plan accordingly.

*To participate fully in caseload supervision and management, to ensure that care plans are matched to need.

*To work with relevant agencies, procedures and legislative requirements

*To work in accordance with all relevant policies and procedures including care planning, risk assessment and safeguarding.

*To deliver evidence-based interventions in accordance with those agreed for use within the service.

*To actively monitor and review the effects of prescribed medication for the patient group noting its efficacy and the effect of potential side effects and acting promptly to escalate concerns or seek advice regarding potential medication changes.

*To administer prescribed medication where required including depot medication in accordance with agreed Medicines Procedures.

*To promote health and wellbeing including smoking cessation, attendance for physical health checks, and encourage healthy activities.
