Job summary

We are seeking a full time qualified mental health practitioner to join the mental health staff in the homeless healthcare team in central Southampton. This could be a mental health nurse, occupational therapist or social worker.

The homeless healthcare team is particularly dedicated to supporting people who are homeless, at risk of homelessness or in temporary accommodation. We are looking for an enthusiastic and creative person who is committed to maximising life opportunities for this client group.

The work is varied and challenging, and there is a strong team approach ensuring good support for the successful applicants. You will need to have a flexible, multi disciplinary approach, have a solid understanding of risk assessment, crisis planning and the recovery model. You will need to be committed to a more outreach way of working and show some awareness of the issues that affects the homeless population. Some knowledge of substance misuse issues, psycho-pharmacology or Psycho-Social Interventions would be advantageous.

Use of a car is an essential requirement of the post.

Main duties of the job

The role is to provide good quality assessments and interventions to service users who have severe or enduring mental health problems and who face homelessness. Support should also be offered to their families and carers.

*To provide a care coordination role as a member of a multi-disciplinary team, engaging, supporting and working with a caseload of service users with severe or enduring mental health problems who are also either homeless or living in temporary accommodation.

*To help service users manage their symptoms, deliver evidence based interventions and give advice and support on practical issues such as housing, benefits, education, employment, medication and social activities in the local area.

*To assess and monitor service user's mental state, provide comprehensive risk assessment and implement management plan, undertake crisis management and contingency planning.

*To establish supportive partnerships with both service users and the family / social networks. This includes close working with staff from Solent NHS trust who look after the physical health care of the homeless population and also the Two Saints day centre. This post involves co-working with staff from a range of voluntary organisations including the hostels and the substance misuse services.

*To participate in weekly team meetings, communicating service user needs and be able to assist in prioritising the allocation of workload.

*To contribute to the weekly multi-disciplinary meeting where service users' needs are discussed.

*To formulate care plans in line with identified need and review in line with the Trust Care Programme Approach and Care Planning Policy.

*To provide therapeutic interventions in line with the agreed care plan and risk assessment, utilising psycho-social interventions and work on improving physical health.
