Job summary

We are seeking an experienced manager with excellent clinical skills and an ability to engage young people with mental health difficulties, families, system partners and stakeholders, to join our recently established Mid and North team. Applicants must have experience of working with service users with mental health difficulties, leadership skills which will support the operational management of our intensive home treatment service, and a style of working which supports all team members to achieve the best possible outcomes for young people and their carers.

The Closer2Home service operates across Wessex and Dorset, provided by four NHS Trusts, working in partnership with the Wessex and Dorset Tier 4 Provider Collaborative (W&D PC) to deliver a clinical model of evidence-based intensive home treatment as a safe and effective alternative to hospital admission.

Based out of Leigh House near Winchester, with the opportunity to work from home when work tasks permit, the Team Manager will be responsible for the delivery of all aspects of the service in the Mid and North team, and also work closely with the Closer2Home Service Manager, Lead Clinician and Quality Lead, as well as the Team Managers from the other Trusts, to provide a fully integrated service across Dorset and Hampshire.

The Closer2Home teams will work together to deliver a specialist service to help young people who otherwise would have been admitted to an inpatient hospital to remain at home.

Main duties of the job

The Closer2Home Team is a Children and Young Persons Mental Health Intensive Home Treatment service that supports young people and families in the community as an alternative to an inpatient admission.

The service operates across Wessex and Dorset, provided by five NHS Trusts, working in partnership with the Wessex and Dorset Tier 4 Provider Collaborative (W&D PC) to deliver a clinical model of evidence-based intensive home treatment as a safe and effective alternative to hospital admission.The role of Team Manager is to manage all aspects of the C2H service, taking responsibility for the delivery of the C2H Team for the Trust you are employed by, but working together across Trusts to provide a fully integrated C2H service across Dorset and Hampshire. A memorandum of understanding is in place between partner Trusts to ensure a truly integrated service and to ensure a 'one team' approach to the delivery of the service.

You will be required to work closely with a large range of internal and external stakeholders, and in particular the W&D PC to manage referrals to / from the C2H service.

*To work with the C2H Team Mangers from the other Trusts to jointly manage demand and capacity across the whole C2H service, and co-ordinate recruitment and training activities within and between Trusts.

*To work effectively with the W&D Provider Collaborative Case Managers, inpatient providers and other staff as required to provide an integrated response to Tier 4 demand.

*To work effectively within a multidisciplinary setting, holding self and colleagues to account appropriately and demonstrating personal commitment to integrated professional and managerial leadership across the C2H service, with particular responsibility for the C2H Team provided by your employing Trust.

*To line-manage Band 6 / 7 Mental Health Practitioners who may be from a range of professions, including nursing, social work, psychology and other allied health professions.

*Achieve an agreed set of performance targets.

*Provide services within the agreed financial resources and achieve all financial targets, including cost efficiency savings ensuring maximum value in terms of clinical and cost effectiveness.

*To contribute to the planning and delivery of the service model, managing change, ensuring patient focus, consistency, quality, and cost effectiveness.

*Identify risks and take corrective action in line with the W&D PC and your employing Trust's governance frameworks.

*To promote and display via personal actions support for the Trust's key policies on Equality and Diversity, Exemplar Employer and implementation of the CAMHS transformation agenda.

*To develop and deliver training, education and development opportunities for staff across the C2H service

*Working with the C2H Service Manager, Lead Clinician and Quality Lead, support the promotion of clinical innovation and delivering continual development of clinical practice based upon evidence and relevant research and that service delivery is based on NICE guidelines and other relevant documents and guidance.

*To ensure all professionally registered staff have mechanisms that ensure they comply with their Code of Professional Conduct and regulation requirements as set out by their respective professional body.

*To act as point of contact for all Safeguarding requirements relating to the C2H Team provided by your employing Trust, liaising with the wider C2H service and W&D PC as required.

*To provide expert advice to patients, carers and staff.

*Deputise for the C2H Service Manager and other colleagues within the Trust and W&D PC as required.

*Providing on-call / cover for the service during the extended hours of operation

  • Educated to degree / diploma level with a relevant qualification or equivalent experience.
  • Clinical / post graduate leadership and management qualification or equivalent level of experience and proven ability to perform at this level
