Job summary

An exciting opportunity has arisen for a Band 6 Care Co-ordinator working within the Early Intervention in Psychosis Team based at the Elms, Tatchbury Mount. The role will involve working across the South West Hampshire area. The work is varied and interesting and there is a strong team approach ensuring good support for the successful applicant. The role requires lone working, use of initiative and a commitment to team working and service user empowerment. There will be an opportunity to develop your skills working in a specialist team and to increase your knowledge of working with psychosis. The post holder will be from a mental health nursing background. You will need to be able to work flexibly and to demonstrate knowledge of the CPA process, risk assessment, and the recovery model. This post may involve weekend and bank holiday working on a rota basis as per service user need.

Main duties of the job

To provide psychiatric interventions to clients presenting with first episode psychosis and their families/carers within the South West Hampshire area, and specifically to help ensure the provision of a comprehensive service to those made most vulnerable by virtue of mental ill health

Job description

Job responsibilities

To maintain a high standard of professional practice in the assessment of care needs, treatment and evaluation of programmes of care.

To attend and participate in C.P.A.s, and other joint meetings, to identify suitable clients for treatment. To ensure the provision of a C.P.A. for all clients.

To undertake initial team assessments of referred clients and report to the referral agents after discussion/supervision from the appropriate Team Meeting. To also undertake joint assessments with colleagues from other disciplines and agencies as and when appropriate.

To participate/lead in the CPA and become a care manager for an identified caseload.

To formulate and implement treatment programmes and report to the referring agent on the client's progress.

To complete an accurate assessment of risk with all new clients and provide the initial management plan. All risk management plans will be reviewed and updated as appropriate.

To agree and plan final discharge providing reports for the referring agent when treatment programmes are completed, and when community team involvement is no longer considered necessary.

To support the team in carrying out physical health checks for all individuals on caseload.

To see clients within other health care settings eg. GP practice where this is felt appropriate.

To participate in the audit of clinical work to focus in particular on quality and outcome issues.

To work within the agreed policies of the Directorate/Trust ensuring that all nursing staff who work or are allocated, are familiar with and comply with their requirements, eg. Child Protection and Vulnerable Adult Policies.

To abide by the District Policy for the Control and Administration of Medicine. To keep regularly updated regarding changes in Policy and Medication.

To safely administer depot medication, where required.

To have an appropriate understanding of legislation relating to work in the psychiatric setting, and in particular the Mental Health Act 1983, and all other relevant legislation.

To provide Social Circumstances Reports for Mental Health Act Tribunals and attend as required by the Team Manager.

To work in collaboration with other mental health professionals and recognise and respect their particular contribution within the mental health team.

To combine with other members of the team, either in conjoint therapy or case work as appropriate. Where appropriate, to take the lead in working with Dual Diagnosis clients. eg. Drug/Alcohol and Learning Difficulties.

Where appropriate, to communicate and liaise with other agencies involved in providing health care and social care.

a)To promote and maintain a close working relationship with medical staff, other nurses and therapists.

b)To assist in establishing and maintaining lines of communication with wards, and hospital departments, G.Ps, voluntary agencies and other organisations.

c)To be aware of and liaise with therapeutic and supportive facilities in the community as appropriate.

d)To continue to support carers and relatives and engage in carer support and family work interventions.

To contribute to multi-professional assessment and treatment.

To liaise with ward staff/crisis team and provide care co-ordination to clients in hospital

To participate in meetings and to offer suggestions on the development of the Mental Health and Community Psychiatric Service.

To participate in the periodic evaluation of the role.

To abide by relevant Professional Code of Conduct.
