Job summary

Mental Health has received significant investment as part of the Community Mental Health transformation programme which has enabled the development of the mental health care offer within primary care. This is in line with the Long Term Plan for Mental Health and the No Wrong Door agenda.

This post is part of a multidisciplinary team and based within primary care and supported by the Basingstoke Community Mental Health Team.

The successful candidate will be able to provide clinical and managerial leadership to the day to day operation of the primary care mental health team, provide direct clinical care to primary care patients, support to staff within the Primary Care Network and be engaged with the local community mental health team alongside all other partner agencies.

You will need excellent assessment and communication skills. As a senior clinician, you will need to work alongside other agencies to promote holistic recovery focused care as well as producing reports, completing audits, supervising staff and running clinics as necessary.

The successful candidates will have sound clinical skills, be able to coordinate complex care packages, have good administration and IT skills, as well as a good working knowledge of Care Planning processes, report writing, organisational skills, safeguarding knowledge and the ability to identify issues and demonstrate ownership of change processes.

Main duties of the job

To provide clinical and managerial leadership to the day to day operation of the integrated clinical team, monitoring workload, allocating duties and ensuring duties are discharged safely and effectively.

To supervise the day-to-day work of the clinical team and provide expertise and guidance in complex and high risk cases, in line with Trust and Local Authority policies and procedures.

To champion a culture based on principles of recovery, choice and personalised care, providing a role model to the rest of the team.

To take a proactive approach to caseload supervision and management to ensure that practitioner skills are matched to clinical need, and that service users are able to flow through pathways of care.

To deputise for the PCN Manager as required.

Management:

To be responsible for the day-to-day management of the clinical service, working closely with the CMHT Manager and PCN Manager.

To supervise and manage the daily workload in such a way to maximise efficiency, ensuring that duties are discharged safely and effectively. This includes the processing, evaluation and allocation of referrals as appropriate to the team's function, and an efficient caseload management system.

To be available to practitioners for advice, guidance and support in the discharge of their duties, ensuring the team works within the care planning policy and risk policy.

To ensure that care planning reflects agreed clinical care pathways (currently under development) and personalised care planning.

To line manage an identified group of practitioners, including supervision, appraisal and development.

To undertake investigations in line with the relevant organisational policy (e.g. complaints, CIRs, HR investigations)

To identify and address poor performance in line with organisational policy.

To alert the CMHT or PCN Manager in a timely manner to any concerns about the team's effectiveness, or that of any of its members.

To lead interagency processes on behalf of the PCN and CMHT Manager in complex and/or high risk cases, such as safeguarding, case conferences etc.

To be involved in recruitment and selection.

To act as an authorised signatory for invoices, salary claims and employment changes.

To work within agreed budgets NHS & LA.

To deputise for the PCN Manager as required, and lead on agreed areas of work for the team within the PCN.

Quality Governance

To play a key role in ensuring the team is complaint with CQC standards.

To contribute to a culture which supports learning and improvement.

To ensure agreed service standards are upheld and monitored, through the use of audit, benchmarking, user feedback etc.

To use data to inform service evaluation and improvement.

To share best practice with the team and ensure it is embedded in day to day practice.
