Job summary
Due to the exciting transformation of the Memory Assessment Service we are currently seeking highly skilled, motivated, passionate practitioners with a desire to provide high quality, timely care.
Recruiting across Mid Hants, if you have a desire to be to part of innovative service re-design and expansion then we are looking for you.
You may already be a B6 or looking for your next step. We are keen to hear from nurses, occupational therapists or any other professionally registered clinician with a desire to work with individuals who require a memory assessment.
** We are also offering £2,500 Golden Hello Payment to external candidates from outside of Hampshire and Isle of Wight OR an exciting opportunity to be supported to complete further study which may include the prescribing pathway.**

Main duties of the job
The Senior Community Mental Health Practitioner will work as a core member of the Memory Assessment Service patient journey pathway ,supervising Band 5 junior team members where appropriate.
Working under the supervision of the Manager/Specialist Practitioner the team member will formulate care and risk management plans in accordance with identified need and ensure that these are recovery focussed and developed in collaboration with the service user and where appropriate their relative and carer.
Working under the supervision of the Manager/Specialist Practitioner the team member will undertake memory assessments as appropriate.
