Job summary

Calling all Mental Health, RCN and LD Nurses. Come and be part of our friendly and supportive team within Gosport War Memorial Hospital.

You will be a registered Mental Health, RCN or LD nurse with relevant experience, who is able to demonstrate a passion for the care of patients and their families.

Main duties of the job

The Mental Health Nurse in Inpatient Services is a core member of the ward team. They will act as a 'named nurse' for a specific group of inpatients on the ward and take responsibility for the assessment, planning, implementation and evaluation of their nursing care needs.

The nurse will take clinical leadership of the ward environment on a shift by shift basis as required working in a way that promotes the principles of recovery to ensure individualised, safe and effective care provision for all patients. In the absence of the ward manager/deputy they will also take the necessary managerial action to ensure compliance with organisational and stakeholder requirements.

Job description

Job responsibilities

To act as the 'named nurse' for a specific group of patients and take responsibility for the assessment, planning, implementation and evaluation of their nursing care needs.

To work effectively with other members of the multi-professional team in the planning, delivery and evaluation of care.

To ensure that junior staff and learners are actively engaged in delivering patient care in accordance with the care plan.

To observe, report and record changes in the patient's physical and mental health wellbeing and ensure timely escalation to appropriate members of the multi-professional team where this is required.

To maintain a sound working knowledge of the legislation, policy and guidance necessary to care effectively for older people with mental health needs.

To identify, provide and supervise the necessary physical care to service users to help them complete their daily living activities i.e. personal hygiene, diet and fluid intake, clothing and laundry, patient handling, personal environment, elimination, maintaining rest and sleep, physical observations and activity.

To undertake complex physical nursing care interventions where these are indicated in accordance with the plan of care and agreed nursing and medicines procedures e.g. wound care, administration of subcutaneous or intramuscular injection, setting up of subcutaneous infusion pumps.

To identify, provide and supervise the necessary therapeutic activities, emotional support and stability to patients that ensure individualised and effective engagement whilst maintaining professional boundaries.

To create and maintain professional supportive relationships with all members of staff and with other professionals and agencies to enhance recovery.

To undertake prescribed physical and mental health observations as required reporting exceptions as appropriate and using tools available e.g. SBAR, Track & Trigger

To ensure the appropriate management and administration of medications in accordance with Legislation, Trust Policy and Professional Standards.

To actively monitor and review the effects of prescribed medication for the patient group noting its efficacy and the effect of potential side effects and acting promptly to escalate concerns or seek advice regarding potential medication changes.

To work with relevant agencies, procedures and legislative requirements in particular the NHS and Community Care Act, Health & Social Care Act, the Mental Health Act, Data Protection Act, Mental Capacity Act, Adult and Child Protection Procedures.

To take responsibility for their delegated duties under the Mental Health Act.
