Job summary
Our CMHT is a lively and dynamic Multi-disciplinary team, consisting of Social Workers, Nurses, Psychological Therapists, Support Workers and Consultant Psychiatrists. Our work is recovery focused and partnership based. The team is proactive regarding the care of service users with a "can do" approach to the provision of Community Mental Health Services for the local population.
The role is varied with no 2 days the same. You will need excellent assessment and communication skills. You will carry a caseload and coordinate their care and participate in the teams shared care function- supporting service users who need a higher level of clinical contact. As a senior clinician, you will work alongside other agencies to promote holistic recovery focused care as in addition to providing reports, quality improvement and supervising junior staff and running clinics where appropriate.
You will demonstrate that you are experienced in care coordination, assessments, risk formulation and management. You will provide evidence based interventions, medication management, provision of reports. You will have a sound knowledge of CPA processes and safeguarding with the care of our service users being our primary focus at all times.
You will need to be able to work autonomously in the community and use your own initiative and you will also undertake working from home at times if desired.

Main duties of the job
To provide recovery based community mental health services within a multidisciplinary team to adult individuals with severe and enduring mental health needs referred to the Community Mental Health Team. This role, in collaboration with referred individuals and where appropriate family or carers, will coordinate and provide recovery focussed assessments, risk formulations, care planning, crisis and contingency planning, evidence based interventions, response, reviews, reports, discharge planning and attendance at appropriate meetings. Role further includes acting as clinical and case management supervisor and mentor.
You will also provide mentorship and supervision to junior colleagues and deputise for the Team Leader as required.
