An exciting opportunity has arisen within the Chandlers Ford Therapy Team to appoint an enthusiastic and motivated Occupational Therapist.
We are seeking a Band 6 Occupational Therapist to work within our Integrated Community Team, visiting people in their own homes or clinic setting. We provide an Urgent Community Response model of care to prevent hospital admission whilst continuing to deliver rehabilitation to adults living in the area who have long-term or complex health needs.
You will work closely alongside the nursing and frailty teams and wider Primary Care Colleagues to prevent hospital admissions, support patients being discharged from hospital, palliative care and promote self-management for patients with long term conditions.
We are looking for a skilled and motivated practitioner who is dynamic and keen to work as part of a team to achieve the best for their patients. The successful candidate will need to have good communication skills, the ability to work well within a team and be able to demonstrate good time management and prioritization skills.
The post holder will participate in the development, training, and support of the multi professional team and students. In return Southern Health offer training opportunities to support ongoing career development.

Main duties of the job
*To provide specialist therapy skills within the Integrated Community Team working both in the community and 9 bedded inpatient unit.
*To assess and treat own caseload, which may include complex presentations and to maintain own records as an autonomous practitioner.
*To assist senior staff in the education and supervision of junior colleagues, assistants and students.
*To assist senior staff in daily management of the team and provision of therapy service, deputising for the team leader in their absence.
*To assist senior staff in development and maintenance of an efficient and effective therapy service.
*To contribute to the inter-disciplinary working within the Community therapy team and Locality by teaching, learning & practicing inter-disciplinary skills & techniques as defined by an agreed competency framework or training scheme.

To provide specialist assessment of patients, using analytical and judgment skills. To provide appropriate patient centred treatment using evidence based practice where-ever possible. Patients will present with acute or chronic conditions and complex multi-system pathologies e.g. neuro, respiratory conditions, orthopaedic rehabilitation and age related deterioration.
To devise effective care plans for each patient, recognizing him or her as an individual. The plan of care, which has been developed in conjunction with the patient, carer and relevant others, should be outcome based and ensure appropriate pathways of care and communication via liaison and referral to other agencies as required
The goals and objectives of any intervention are clearly established and negotiated, and where appropriate can be assessed through use of outcome measures/ objective markers.
To provide the treatment programme or where appropriate direct the intervention as necessary through Health Care Support Workers, or other members of the multi-disciplinary team.
To provide a regular system of monitoring patients, whose treatment has been delegated to a Health Care Support Worker.
To advise, support and empower patients and their families/ carers in order to promote understanding of their condition, aims of therapy and aid self-management where appropriate.
To effectively communicate verbally and non-verbally with patients and carers in a range of situations. This will include overcoming frequent barriers to communication experienced by patients with complex emotional, physical and psychological barriers, e.g. confusional states, expressive and receptive dysphasia, depression, hearing and visual impairment, altered state of consciousness or patients with significant perceptual and cognitive impairments.
To communicate effectively to provide advice regarding therapeutic management of patients to the MDT and work collaboratively with the MDT to deliver patient-centred services. This includes regular active participation in ward rounds, multi-disciplinary meetings and discharge planning.
To facilitate safe discharge at the optimum point of care, including assessing risk, ordering equipment and providing appropriate advice for patients/ carers.
To ensure appropriate arrangements are made for the transfer of care, including transfer to inpatients.
To provide transfer/discharge reports on the patient's management to relevant agencies.
To undertake managerial and clinical supervision with band 5's, Health Care Support Workers and students.
To support Band 5's and other members of the MDT in developing skills to deal with complex cases, by providing specialist knowledge and practical skills through supervision, provision of and in-service training program and formal appraisal.
To maintain accurate and up to date patient records as an autonomous practitioner and in accordance with legal requirements, Trust standards and Professional Standards. To use RIO as appropriate.
To be responsible for organising and planning of own caseload to meet service and patient needs with support of senior as required. This will include the need for regular prioritisation skills and flexibility to meet the varied and changing demands of the service on a daily basis.
To deputise for the team leader as required during annual leave, sick leave etc.
To assist the team leader in planning and delivery of a seven day therapy service, as appropriate.
To work in various community locations, as agreed with the line manager, as per patient needs and service requirements.
To submit accurate and timely statistical information on patient activity as required by the service/ Trust.
To participate in the collection of data, undertake literature searches and reviews etc in support of evidence based clinical audit in the relevant field of interest.
To contribute to interprofessional working by teaching, learning & practicing interprofessional skills & techniques as defined by an agreed competency framework or training scheme. Where appropriate to take responsibility for the application of those new skills e.g. - equipment provision, cognitive assessments and environmental assessments.
