Job summary
The Patient Pathway Team for Southampton Division are looking for an experienced social worker who is motivated, can work under pressure and is able to demonstrate the benefit of having social work within an acute mental health setting supporting discharge from our acute inpatient hospital as well as the community rehab.
Under the direction of the Team Manager, you will lead a small team of Band 4 Care Navigators working in a targeted way to achieve the discharge planning for inpatients, maximise independence and choice for individuals, and work in a solution focussed way to ensure that there are no barriers in place to an effective discharge.
Working closely with the inpatient teams you will have patient facing contact, attend CPA's, professionals' meetings and take a lead or advise where there are safeguarding or other concerns.
You will also work with individuals, families and carers, community services, housing departments, social care, third sector organisations, and other relevant parties to ensure smooth transition to the community from hospital.
There will be an expectation of supporting the team in the managers absence attending meetings as well as producing performance related reports related to patient pathways.

Main duties of the job
To deliver Social Work intervention to support adults with severe and enduring mental
health issues and their care team in Southampton. To ensure personalised support
that meets statutory legislation e.g. Mental Health Act, Mental Capacity Act Best
interest meeting, attending MARMS, safeguarding, supporting the local authority with
care and support plans and Care Act.
To ensure that referral, assessment, support plans and reviews are undertaken to
meet HCPC professional standards. Interventions to include prevention and early
intervention, short term and urgent work, safeguarding and longer term support
planning.
To support the flow of Patients care and to prevent patient from becoming blocked and
delayed discharges from hospital.
To take a lead across the Southampton division with people that are hard to place or
super stranded.
To have strong working relationships with Southampton city council and Rehabilitation
services.
To lead on flow with Forest Lodge patient's pathways of care and supporting
community teams with referral and assessment for social care needs to move on from
health services back into the community or supportive living. Attend weekly MDT and
support a Patient flow meeting with clinical staff.
To provide clinical and managerial leadership to the day-to-day operation of the social
care need in the patient flow team and Southampton rehab services.

To supervise the day-to-day work of the care Navigators, and provide expertise and
guidance in complex cases, in line with the Local and Directorate policies and
procedures. Support the care Navigators with referrals of NFA or duty to refer to
homeless team.
To champion a culture based on the Trust's values providing a role model to the rest of

To provide an innovative and progressive attitude to the continual improvement of the
patients journey thought acute care services, rehabilitation services and community.
Provide a consistent source of senior
skills and support to the wards and
medical teams, psychology and
occupational therapy with regards to the social care needs of patients working towards
moving on from acute care and inpatients rehabilitation services.
To ensure all staff understand the importance of providing timely bed availability
information to the Patient flow Team and working in a time affective SMART way.
To provide relevant up to date data regarding bed management to senior line
managers when required.
To take a lead role on all patients that have been in hospital more the 32 days and
support problem solving with social care issue to support care moving on from
inpatient acute care.
To deputise for the Journey and Rehabilitation Matron as required.
To work in partnership with patient flow team leader in supporting pathways of patients
care.
To work with colleagues in managing referrals, undertaking assessments and ensuring
that appropriate advice and information is given to clients and their carers.
To work creatively with the person and their carers to implement support plans that
meet their needs.
To ensure a multi-agency approach is taken, to assess need and manage risk and
complexity.
To support the person to live independently in their community with treatment, tenancy
support and networking. If a more intensive support plan is required, to consult with
the Council's Placements Team.
To supervise and manage the daily workload in such a way to maximise efficiency,
ensuring that duties are discharged safely and effectively.
To be available to practitioners for advice, guidance and support in the discharge of
their duties.
