We are looking for an enthusiastic Band 6 Discharge liaison practitioner to join our friendly multi-disciplinary team. You are keen to hear from you if you are a qualified nurse, social worker or occupational therapist.

Main duties of the job
To support the safe discharge of individuals who have been admitted to an acute mental health unit.
Providing a 3-month period of monitoring in the community with a clear care plan.
Reviewing mental health needs and providing focussed interventions to support that recovery.
Enabling a safe transfer of care to primary care as appropriate.
Referring for CMHT Care Coordinator allocation if longer-term mental health needs are identified.
Making onward referrals and/or signposting to appropriate agencies.
The post holder will be expected to work proactively in forging professional working relationships with multi-agencies.
Car driver is preferrable. You will be working 9am -5pm Monday to Friday and occasional weekend or bank holidays. We are flexible regarding working patterns and working hours.
