They will also work with partner agencies, such as Hampshire, Southampton and Isle of Wight CCG, and Hampshire County Council.

Main duties of the job

The Community Team is based at Sterne 1-3, Tatchbury Mount, Calmore. The post involves travelling around the area the team covers, but technology is also being used appropriately to limit travel. However, home visits are necessary, and are always arranged appropriately.

There is a Consultant Psychiatrist within the team. The team offers nursing, psychology, occupational therapy, physiotherapy and speech and language in-put. It provides specialist health advice to service users, their families and car-ers in partnership with a wide range of agencies. There is also access to a Hampshire wide Intensive Support Team.

All routine cases are assessed by a telephone screening, and a clinical assessment is undertaken, allocation to psychiatry usually takes place after team discussion. Work is currently being undertaken to look at a Single Point of Access for all LD services. The team takes a holistic approach to the assessment and management of our patient population and the post holder would be expected to work to this ethos.

The team accepts referrals from a range of sources including the service users, their carers or primary care. The majority of service users are seen in clinic with a smaller group being offered community visits. Services provided are centred on 7 areas of clinical practice: mental health, dementia, challenging behaviour, autism, forensic, epilepsy and complex health needs.
