Job summary
Would you like to work in a team that predominantly works 9-5 Monday to Friday but can offer flexible working hours?
We are looking for an experienced individual to complete initial assessments and support service users on shared care. Our shared care service supports service users who require increased support for a short period but do not require crisis intervention.
The Winchester Adult CMHT is looking for enthusiastic, dedicated, and talented professional to join the team. To fulfil the requirements of this role you need to hold a professional registration in Social Work, Nursing or Occupational Therapy and be competent in completing high quality assessments of individual's needs. The successful applicant will be expected to participate in the teams "shared care" rota and complete initial assessments.
As the successful candidate will be working alongside other agencies to promote holistic recovery focused care excellent communication skills are a must.

Main duties of the job
To be an active member of the multi-disciplinary team supporting the assessment and care planning for our service users.
To act as care co-ordinator to an agreed caseload of service users with severe and enduring mental illness.
To undertake ongoing assessment, treatment and evaluation for service users known to the service
To support the delivery of therapeutic interventions either individually or within a group setting.
Support the transformation of the team to deliver local evidence based care
