The Meadows CMHT is a busy, supportive environment which is caring to staff and keen to allow and bring new and positive change -- we welcome bright new ideas. It is a dynamic multi-disciplinary team which provides care coordination and recovery focused care for individuals with complex and challenging mental health needs.

Main duties of the job

To be an active member of the multi-disciplinary team supporting the assessment and care planning for our service users.

To act as care co-ordinator to an agreed caseload of service users with severe and enduring mental illness.

To undertake ongoing review, review of high cost placements, treatment and evaluation for service users known to the service.

To be a safeguarding lead in the CMHT. Seeking support and advice from the SHFT safeguarding team.

Support the transformation of the team to deliver local evidence based care.
