Job summary
An exciting opportunity has arisen within Southern Health NHS Foundation Trust to appoint inspirational and enthusiastic Community Nursery Nurses, to join the Gosport Health Visiting Teams The post holder may be required to support other teams in the South East with the delivery of the Healthy Child Programme core contacts.

We are looking for a motivated professional with excellent communication skills who has the ability to work independently and as part of a corporate skill mixed team delivering the Healthy Child Programme (HCHC, 2009) using a public health approach.
As a team player, you will work under the leadership and supervision of the Health Visitors. Your role will require excellent assessment skills, partnership working and a strong commitment towards achieving the outcomes of 'Every Child Matters'. You will be expected to demonstrate knowledge and understanding of all aspects of safeguarding children whilst being supported by robust organisational supervision, guidelines, and policies.
With a positive approach to customer care, you will need to be able to demonstrate excellent communication skills, prioritise your workload and manage your time efficiently.
Main duties of the job
To work within a Health Visiting Team to deliver delegated components of the 0-5 Healthy Child Programme (HCP), (DH 2009), monitoring quality and compliance and reporting risks by exception to the Clinical Team Leader.
To promote child health and parenting skills for individual families and groups by participating in planned programmes under the supervision of a Health Visitor in line with the 6 High Impact Areas including: Transition to Parenthood and the Early Weeks including infant attachment, Maternal Mental Health, Breastfeeding, Healthy Weight and Healthy Nutrition, Managing Minor Illness and Reducing Accidents, Health, Wellbeing and Development of the Child Age 2 -- Two year old review.
To work within Hampshire Safeguarding Children's Board (4LSCB June 2018) and the Trust Safeguarding Policy and Procedures (March 2017) (SH CP 56)
To contribute to interventions identified through the child protection plan as delegated by the Health Visitor, and support children and families where there are identified vulnerabilities.
To deliver a safe and effective service using service user feedback to improve patient experience and outcomes.
To assist in programmes of care for children 0-5 years with complex health needs.
To support Health Visitors with integrated working; developing and maintaining relationships with statutory and non-statutory agencies.
