Job summary

To be an active member of the multi-disciplinary team supporting the assessment and care planning for our service users.

To act as care co-ordinator to an agreed caseload of service users with severe and enduring mental illness.

To undertake ongoing review, treatment and evaluation for service users known to the service

To support the delivery of therapeutic interventions either individually or within a group setting.

Support the transformation of the team to deliver local evidence based care.

Main duties of the job

To co-ordinate the care of an agreed caseload of service users with severe and enduring mental illness.

To formulate care plans in line with identified need with service users, carers and other agencies.

To provide recovery focused therapeutic interventions in line with the agreed care plan and risk assessment.

To assess risk, develop risk management plans, review and adjust in line with the service user's needs.

To work in collaboration with service users as experts in their own care, promoting the principles of recovery, choice and personalisation, which value and empower the service user.

To use tools which promote user empowerment (my crisis and safety plan)

To be proactive in ensuring service users receive the range of interventions they need in line with their care plan, whether or not the postholder provides those interventions. This will include physical health monitoring.

To have a proactive approach to the role and needs of carers/families and implement the triangle of care.

To be innovative and responsive to individual needs tailoring care accordingly.

To be proactive in monitoring mental health needs and respond appropriately if needs change.

To participate fully in caseload supervision and management, to ensure that care plans are matched to need and service users are able to 'flow' through pathways of care.

To work in line with all relevant policies and procedures, including care planning, risk and safeguarding.

We are keen to develop are teams to work cohesively with the primary mental health care offer to improve the patient of the journey, which we know improves the satisfaction of our staff.

We are also undergoing investment into training modules and offer a variety of pathways to support you. You will have access to the trust revalidation portfolio, have opportunities to develop your leadership and management skills through dedicated programs, benefit from an excellent library service and enhance your clinical skills through in-house bite-size skills sessions, reflective practice and a journal club. Your training and development is something that we value and discuss during yearly appraisals and regular supervision. We are committed to your development and career progression.
