Job summary

We are seeking to recruit an enthusiastic, and clinically confident Specialist Perinatal Mental Health Nurse to join our Community Perinatal Team. Experience of working in the Perinatal Mental Health field is desirable; a keen interest in this specialist area is essential. Registered Mental Health Nurses, Dual Trained Nurses with a Mental Health field, Occupational Therapists & Social Workers may apply.

We also offer a comprehensive preceptorship programme for newly qualified registered Mental Health Nurses

This post will offer provision in the Winchester and Andover geographical areas of the Hampshire & IOW service with flexible working between home and base. Car driver and use of a car within work hours is essential.

We are commissioned to work with women/birthing people pre-conceptually, in pregnancy and postnatally where there are severe and complex mental health concerns. With the increase in several clinical roles across the service, we are able to extend our offer for women/birthing people from 12-24 months as well as offering fathers/co-parents screening and signposting for their own mental health needs. The service also provides consultation for advice, information, and signposting for women with a less severe mental illness in the perinatal period to primary care providers including GPs' Health visitors and maternity services as well as the women themselves.

Main duties of the job

The Perinatal community mental health nurse/practitioner plays a pivotal role in assessing, planning, and coordinating women's care whilst under the service, working alongside a multidisciplinary team of Psychiatrists, psychologists, occupational therapists, social workers, Nursery nurses, Peer support workers

Provide specialist advice/support to professionals and mothers regarding safe medication usage in pregnancy and whilst breastfeeding.

Demonstrate a commitment to offering mothers with perinatal illness choices in dealing with their illnesses including a range of therapeutic interventions

Assess the impact of a parent's mental Illness on the baby and older siblings within the individual household, notifying relevant agencies of concerns if appropriate.

The Perinatal community mental health nurse/practitioner will attend, participate and in some cases lead clinical/patient-related meetings such as professionals' meetings, CPA, and MDT.

To assess partners/fathers' mental health needs where screening has highlighted unmet well-being/mental health needs and signpost appropriately where needed

To involve patients in goals and treatment proposals. To gain valid informed consent. To work within the legal framework with patients who lack the capacity for informed consent or are subject to the Mental Health Act.

To develop comprehensive discharge and contingency plans with relevant professionals to arrange ongoing care.

Job description

Job responsibilities

*The Perinatal community nurse/practitioner will be committed to attend service wide meeting agenda (e.g., business team meetings, "keeping baby in mind", safeguarding supervision, education slots) and represent the Perinatal Service at other meetings/activities external to the service.

*The Perinatal community nurse/practitioner will meet regularly with their line manager to discuss caseload management and identify other team business issues.

*Maintain clear communication systems, eg: clients' electronic files, letters to clients, their GP, and other professionals so that risk is managed effectively.

*The Perinatal community mental health nurse/practitioner will be responsible for maintaining close links with other services and identifying unmet needs in the perinatal and other services in relation to this client group. E.g., link with local specialist midwives for the geographical area covered to ensure joined up birth/care planning for women who are pregnant.

*To work collaboratively/ensure partnership working with other mental health teams such as CMHT's AMHT's EIP where joint working with patients is indicated. To ensure Care planning meetings (CPA) and professionals meeting are held and documented with complex cases with multidiscipline attendance such as social care, health visiting, midwifery, obstetrics, and mental health.

*The Perinatal community mental health nurse/practitioner works in close liaison with the mother and Baby Inpatient Unit, Maternity mental health service, IAPT and statutory and voluntary agencies.

SUPERVISION

*To attend clinical supervision provided within a range of settings including peer group, specialist supervision (e.g., tailored education slots and psychologically informed supervisions) and reflective practice.

*Provide supervision for junior staff (e.g., Peer or carer's peer support works and nursery nurses) and peer support to other team members

*Provide mentoring for allocated medical students and student nurses/other AHPs on perinatal service placement.

SERVICE DEVELOPMENT / QUALITY IMPROVEMENT

Currently the service is going through a transformational 3-year period on delivering the NHS Longer term plan goals for perinatal services. This includes

Assessing and signposting for fathers/partners of women under the service

Extending working to mothers with infants up to 24 months where specialist intervention is required

Increased access to psychological therapies.

Ensuring an access rate targets set by NHS England.

The post holder will be expected to:

Have knowledge of the above transformation programme and share the services values and goals regarding achievement of the above.

Participate in research and audit projects within the perinatal team programme, develop a lead role interest.

Contribute to the ongoing development of Quality (CQC) standards for the service and participate with the Royal College of psychiatrist's perinatal peer review cycle, helping with the wider team to ensure nationally set perinatal standards of care delivery are met and maintain accreditation status.

Contribute actively to the development of high-quality care in the Perinatal team through team initiatives, membership of working groups in accordance with overall service objectives and Southern Health Foundation NHS Trust Nursing and Clinical Effectiveness strategies.

Work within NICE guidelines, NHS Trust policies and procedures, NMC Code of Professional Conduct and Health and Safety at Work Act.

Develop and evaluate innovative elements of care, e.g., video support groups; tools for enhancing mother - infant relationship; interventions to support other members of the family.
