Job summary

An exciting opportunity has arisen for a full time senior Registered Mental Health Nurse to join an established team that is based at Petersfield's Community Hospital. We are looking at recruiting nurses due to the expansion of the CMHT.

Consideration will be given for the right candidate to be supported in development to Band 6 competencies level from Band 5.

The Bordon and Petersfield Community Mental Health Team operates between the two sites, the main team base being Petersfield Hospital with a smaller base at Chase Hospital in Bordon and some working in the Havant Area. The team is well known for being approachable and open. We support each other and have developed a culture of honesty and integrity where problems are resolved quickly.

The post holder will be based at Petersfield Community Hospital and will need to work flexibly. You will also need to establish good working relationships with the local GPs, Italk service and other community and voluntary organisations. Main duties of the job

To provide recovery based community mental health services within a multidisciplinary team to adult individuals with severe and enduring mental health needs referred to the Community Mental Health Team. This role, in collaboration with referred individuals and where appropriate family or carers, will coordinate and provide recovery focussed assessments, risk formulations, care planning, crisis and contingency planning, evidence based interventions, response, reviews, reports, discharge planning and attendance at appropriate meetings. Role further includes acting as clinical and case management supervisor and mentor.

2.1 Clinical

2.1.1 To effectively co-ordinate the care of a complex caseload of service users with severe

and enduring mental illness.

2.1.2 To undertake initial assessments where this is clinically appropriate.

2.1.3 To formulate recovery based care plans in line with identified need.

2.1.4 To provide recovery focussed therapeutic interventions in line with the agreed care

plan and risk assessment.

2.1.5 To assess risk and formulate risk management plans, review and adjust in line with the

service user's needs.

2.1.6 To work in collaboration with service users as experts in their own care, promoting the

principles of recovery, choice and personalisation, which value and empower the

service user.

2.1.7 To use tools which promote user empowerment e.g. Wellness Recovery Action Plans

(WRAP), Recovery Star.

2.1.8. To be proactive in ensuring service users receive the range of interventions they need

in line with their care plan, whether or not the post holder provides those interventions.

This will include physical health monitoring.

2.1.9 To have a proactive approach to the role and needs of carers / families.

2.1.10 To be proactive in monitoring mental health needs and respond in a timely and

appropriate fashion if needs change adjusting the care plan accordingly.

2.1.11 To participate fully in caseload supervision and management, to ensure that care plans

are matched to need.

2.1.12 To work with relevant agencies, procedures and legislative requirements, in particular,

The NHS and Community Care Act, Mental Health Act, Data Protection Act, Adult &

Child Protection Procedures.

2.1.13 To work in accordance with all relevant policies and procedures including care

planning, risk assessment and safeguarding.

2.1.14 To deliver evidence based interventions in accordance with those agreed for use within

the service.

2.1.15 To actively monitor and review the effects of prescribed medication for the patient

group noting its efficacy and the affect of potential side effects and acting promptly to

escalate concerns or seek advice regarding potential medication changes.

2.1.16 To administer prescribed medication where required including depot medication in

accordance with agreed Medicines Procedures.

2.1.17 To promote health and wellbeing including smoking cessation, attendance for physical

health checks, and encourage healthy activities.

2.1.18 To attend and participate in clinical Team meetings and other Team meetings as

deemed necessary to fulfil clinical duties and responsibilities.
