Job summary

An exciting opportunity has arisen, and we are looking to appoint a committed, highly motivated, enthusiastic and skilled Band 6 Occupational Therapist to join our dynamic team.

We are looking to recruit a Senior Occupational Therapist to provide clinical and professional Occupational Therapy interventions within the community. The post holder will provide support to the Community Care teams, ensuring that the team contributes to the implementation of Southern Health Clinical Governance Strategies.

The appointed person must be a car driver and have access to a car, this is essential for this role.

This is a part time post (30 hrs a week)

Main duties of the job

To provide advanced clinical and professional Occupational Therapy to the Community Therapy Team, and to provide support to the Integrated Community Care team leaders in ensuring that the team contributes to the implementation of Southern Health Clinical Governance Strategies.

Undertake Occupational Therapy assessments of patients with diverse and complex conditions, providing a clinical diagnosis (where appropriate), development, and delivery of individualised treatment programmes.

To support the case management process by providing expert clinical advice and therapy to patients with a variety of complex conditions living in the community who are at risk of deteriorating health that may result in declining quality of life or avoidable hospital admission to secondary care

To provide clinical Occupational Therapy skills within the Community Therapy Team, and all other provider organisations.

To be skilled in assessing and treating a range of complex factors relating to the patient's condition, diagnosis and prognosis

To provide specialist intervention tailored to meet the uniqueness of the individual.

i.e complex manual handling and 24 hour posture management.
