Job summary
Southern Health NHS Foundation Trust provides high quality community health, mental health, learning disability and social care services in the South of England.
We are seeking an experienced and enthusiastic Mental Health Practitioner, to join our friendly and professional team, currently based across two Basepoint locations -- Havant (Harts Farm Way) and Gosport (Aerodrome Road).
Our Crisis Service covers a 24/7 week service covering Havant, Waterlooville, Bordon, Petersfield, Fareham and Gosport, therefore a car driver is essential.
This role will be focused on working within our Crisis and Assessment Team, providing a 4-hour response to service users who present in a self defined crisis, to complete holistic assessments and make a comprehensive decision on treatment required such as Intensive Home Treatment Team or admission to Hospital.
The role is required to provide clinical support to the Safehaven Service, based in Leigh Park 7 days a week, out of hours, alongside MIND and Inclusion. The service provides an out of hour's responsive assessment and treatment facility for service users presenting in a self defined crisis.
Training and partnership working is also available to enhance your learning through brief psychological interventions. As this role requires you have access to a vehicle Southern Health also offer a lease car scheme for community workers (T&C apply).
Main duties of the job
As a member of the Crisis Urgent Assessment and Home Treatment Team, you will ensure that you offer responsive, evidence-based and recovery focused care to the service users.
You will work as part of a multi-disciplinary team, which supports individuals at home who are experiencing an acute mental health crisis and who require intensive input in order to remain in the community, averting hospital admission where possible, whilst also supporting individuals along their journey through the Acute Care Pathway from the hospital environment to home. Our team is made up of Nurses, Social Workers, Occupational Therapists, Health Care Support Workers, Psychology support, medical and admin staff.
