We are currently recruiting within the Community Specialist Palliative Care Team for an experienced palliative care nurse. The post holder will have the opportunity to continue to develop and grow their career within an innovative, collaborative experienced multi-professional specialist palliative care team.
The post holder will develop advanced skills in the clinical assessment and management of palliative patients and will develop the professional knowledge and maturity to work towards autonomous patient assessment that determines a working diagnosis, rationale for initial treatment and appropriate supportive management. This will include the ability to communicate complex and distressing information. Liaise and network across the broad Multi-Disciplinary Team to ensure a coordinated approach is given to providing patients and families with the appropriate specialist palliative care, ultimately with the goal of enabling patients to attain the best quality of life possible.
The post holder will contribute to rota of weekend working including weekends and Bank Holidays providing specialist palliative care advice and support.
A valid driving licence and the use of a vehicle for work purposes is vital.
Informal enquires and visits welcome and encouraged to fully understand the role and the service.
Main duties of the job
The Band 6 Specialist Nurse is responsible for the provision of specialist nursing care for a defined patient/client group, practicing as a core member of the multi-disciplinary team.
They will practice clinically in agreed care settings. The post holder will work across professional, clinical providers of palliative care services as required to develop and support patient-centred seamless care.
The role encompasses clinical, consultative, education, leadership and research functions. The primary concern of the CNS is to bring the highest standards of clinical care to the patient through good symptom management, emotional support and access to appropriate services and advice, seeking support from more experienced members of the Specialist Palliative Care Team as appropriate.
