Job summary
As a Primary Care Network Occupational Therapist, the post holder will work as part of a multidisciplinary team across the practices in the Gosport Primary Care Network (PCN), with the PCNs GPs, other clinicians and staff, as well as staff and clinicians from community, acute, social, and voluntary sectors. Primary Care Networks aim to bring care closer to patients in the community.
We are committed to continued professional development and promoting evidence-based practise to ensure a high standard of care throughout the service. There is an extensive professional development programme aligned against the First Contact Practitioner roadmap to practice and advancing clinical practice. The successful candidate will have regular supervisions with their team lead. We offer a varied local in-service training programme as well as the opportunity to attend TARGET days. We are keen to develop our workforce and the individual will also have annual opportunities to apply for external course funding, including modules at MSc level in line with the service and the applicant personal development goals.

Main duties of the job
The post holder is responsible for the provision of therapy for the Gosport Primary Care Network (PCN).
They will practice clinically across all practices within Gosport PCN.
The role encompasses clinical, consultative, education, leadership and research functions.
The primary concern of the Occupational Therapist in Primary Care is to bring the highest standards of therapeutic care to the patient through good symptom management, emotional support, access to appropriate services and advice. You will work as part of the PCN multidisciplinary team.
Gosport PCN has a vision to develop a frailty team to support frail patients in the locality. Due to this vision a special interest in frailty would be a great benefit to this role.
