Job summary
Southern Health Foundation Trust has an exciting opportunity for a full-time Health and Wellbeing Coach to join Shakespeare Road Primary Care Network.
We provide primary care services for over 13,000 patients in the Popley area of Basingstoke and the practice is in collaboration with Hampshire Hospitals NHS Foundation Trust, Solent NHS Trust and North Hampshire Urgent Care.
The Health & Wellbeing Coach works alongside a team within general practice and within the Primary Care Network (PCN) and empowers people to take control of their health and well-being.
We are looking for a person that can motivate patients to manage their own health & well-being to live independently and improve their health outcomes. The post holder will work with individuals and/or groups of patients to deliver sessions on how to motivate patients to improve their physical activity, eat healthily to meet personalised care plans.
You will use a holistic approach to promote lifestyle changes, support patients to address their existing health problems and prevent new ones.
The role works within their practices multi-disciplinary team, a full induction and training is given to help candidates integrate into Primary Care.
Main duties of the job
Health & Wellbeing Coaches help their clients to assess their lifestyles and wellbeing, set goals for improving their health, agree action-plans, and provide practical support and information that will help people to take an active role in managing their health and wellbeing.
Working as part of the Extended Primary Care Team alongside Health Connectors, the Health & Wellbeing Coach will work across the Shakespeare Road Primary Care Network. Health & Wellbeing Coaches will provide a time-limited intervention to support individuals that have been referred to them from primary care and the Extended Primary Care Team.
