Job summary

An exciting opportunity has arisen for a registered Mental Health Nurse to join the older person's community mental health team, providing care to patients within Southampton. We are a highly supportive and innovative team that aspires to provide a responsive, safe, effective, caring service that is well led. We have posts available at both Western Community Hospital and Moorgreen Hospital.

The post will involve managing a caseload of patients and coordinating and delivering care in conjunction with the agreed care plan. The successful applicant will also contribute to the wider working of the mental health team by taking link roles and contributing to the duty system.

We are looking for a highly motivated practitioner who is able to demonstrate a keen interest in older person's mental health and will be able to demonstrate this through work experience and placements. In return you will benefit from a team of very experienced mental health practitioners, which include doctors, senior nurses, psychologists, and AHPs.

Main duties of the job

The Community Mental Health Nurse/ Practitioner will work as a core member of the Community Mental Health team to co-ordinate the care of an agreed caseload of service users who may require an assessment of their mental health needs under the supervision of a senior practitioner.

1.2The practitioner will formulate care and risk management plans in accordance with identified need and ensure that these are recovery focussed and developed in collaboration with the service user and where appropriate their relative and carer.

CLINICAL

2.1To co-ordinate the care of an agreed caseload of service users.

2.2To undertake initial assessments where this is clinically appropriate and with the agreement of their caseload supervisor.

2.3To formulate care plans in line with identified need.

2.4To provide recovery focussed therapeutic interventions in line with the agreed care plan and risk assessment.

2.5To assess risk, develop risk management plans, review and adjust in line with the service user's needs. This will include providing Memory Matters to service users and their carers.

2.6To undertake detailed assessment under supervision.

2.7To work in collaboration with service users as experts in their own care, promoting the principles of recovery, choice and personalisation, which value and empower the service user.

2.8To use tools which promote user empowerment e.g. Wellness Recovery Action Plans (WRAP)

2.9To be proactive in ensuring service users receive the range of interventions they need in line with their care plan, whether or not the post holder provides those interventions. This will include physical health monitoring.

2.10To have a proactive approach to the role and needs of carers / families.

2.11To be proactive in monitoring mental health needs and respond in a timely fashion if needs change adjusting the care plan accordingly.

2.12To participate fully in caseload supervision and management, to ensure that care plans are matched to need.

2.13To work with relevant agencies, procedures and legislative requirements, in particular, The NHS and Community Care Act, Mental Health Act, Data Protection Act, Adult & Child Protection Procedures.

2.14To work in accordance with all relevant policies and procedures including care planning, risk assessment and safeguarding

2.15To deliver evidence based interventions in accordance with those agreed for use within the service

2.16(Profession specific) To actively monitor and review the effects of prescribed medication for the patient group noting its efficacy and the affect of potential side effects and acting promptly to escalate concerns or seek advice regarding potential medication changes.
