Job summary
We are looking for a motivated and enthusiastic Mental Health Nurse, Occupational Therapist or Social Worker to join our local Older People Community Mental Health Team. This post involves working in Southampton providing skilled assessment and treatment interventions to older people living with mental health problems and the successful candidate will take on the role of a care coordinator for patients. The post is based at the Western Community Hospital and is a substantive post and available full time, part time hours may be considered. We are a very friendly and supportive team.
You must have a genuine interest in working with older people and be able to demonstrate a real commitment to providing high quality and compassionate care to patients and their families. You will play an important role in the team as we aspire to provide excellent patient care and develop our services in line with national and local drivers.
There is a strong multidisciplinary focus within the team and you will be integral to the implementation of programmes of care and evaluating and communicating effectively with other parts of the service.
It is essential you have a driving licence and access to a car.
Main duties of the job
1.1 The Senior Community Mental Health Practitioner will work as a core member of the Community Mental Health team to co-ordinate the care of a complex caseload of service users supervising a Band 5 Community Practitioner/Band 3 Community HCSW where appropriate.
1.2 Working under the supervision of the Deputy Team Manager/Team Manager the practitioner will formulate care and risk management plans in accordance with identified need and ensure that these are recovery focussed and developed in collaboration with the service user and where appropriate their relative and carer.
