Job summary
Our community team provide therapy to a multitude of patients in their own homes, within the Totton & Hythe area of the New Forest. We are a team of registered PTs and OTs with support from our Senior Exercises Instructors and Rehabilitation Assistants.
We pride ourselves on working towards an MDT model with our patients, and the therapy team is part of a wider ONE TEAM. This includes collaborating with other primary and secondary health services such as GPs, Frailty, Community Nursing, OPMH, Consultant geriatricians, Social Services.
As a service our work varies from supporting patients with their discharges home from hospital and devising rehabilitation and/or management programmes of patients with a wide variety of long-term conditions. Along side of general therapy provision, we also provide an Urgent Crisis Response (UCR) service aiming to prevent hospital admission (this runs 7 days a week, 8am-8pm). By working collaboratively with our One Team we can ensure patients remain at home for longer and reduce unnecessary admissions into acute services.
We are looking for applicants who are keen to develop their skills and who are able to provide high quality therapy to their caseloads. You are required to have excellent communication, time management and prioritisation skills, that will enable you to provide care to patients within their own homes. Flexibility in working patterns is desirable as the team covers a 7 day 8-8pm service.
Main duties of the job
The primary role of the Senior Physiotherapist is to provide strong holistic leadership to the team and support the Clinical Team Lead (Band 7). To include day to day leadership responsibilities, development and creative leadership, responsive leadership, enabling the team to achieve a 'well led' culture.
The Senior Physiotherapist in conjunction with a wide range of clinical colleagues and specifically, GP's, Practice Nurses and Social Care professionals, will lead and facilitate a patient or client focused, co-ordinated case management approach across primary and secondary care for people who are most vulnerable to, and at high risk of repeat admissions to hospital.
The Senior Physiotherapist will participate in and influence efforts across health and social services to shape multi-disciplinary pathways designed to support patient choice, improve quality of life, promote self-management and assure early intervention through the proactive provision of care in or as close to the patient's own home as possible.
The Physiotherapist will work across the caseload and the single point of access or (equivalent), using their clinical skills to identify the needs of patients and the correct services to liaise with.
The Physiotherapist will provide an expertise in their own discipline to the wider team.
