Job summary
Seeking a mythical beast or two, Care of the Elderly/Frailty/Rehabilitation Consultant Geriatrician/s wanting to work primarily in community services. Southern Health NHS Foundation Trust, the largest community Physical and Mental Health community provider, is developing its frailty services utilising the sensible bits from Urgent Care Response and Virtual Ward national guidance. We are seeking a Physician keen to sink roots into the Portsmouth and South East Hampshire area. Working across the South East Hampshire division of SHFT you will support and help mold our expanding team of consultants, nurse consultants, specialty doctors, Advanced Clinical Practitioners and community teams. Based primarily at Petersfield Community Hospital you will work alongside the Divisional Clinical Director/Community Geriatrician based at Gosport War Memorial Hospital, providing oversight to 4 wards, 2 Assessment Units, with linked Virtual Wards, and a single LDS Urgent Care response line.
Senior Specialty or Specialist doctors actively progressing CESR applications should consider talking to us. Interests in teaching, training or service development would be welcomed.
Strong links to Portsmouth Hospitals University NHS Trust and funding would allow successful applicants to undertake specialist or acute sessions as part of a job planned portfolio, should they wish.
Petersfield station is 400 metres from the community hospital, with direct links to London.
Main duties of the job
When working on the Community Hospital ward
* Responsibility for leadership of the day to day medical care of inpatients will remain with the nurse consultant.
* Conduct at least one consultant ward round each week to ensure optimum specialist medical management of inpatients.
* Advising Specialty Doctors and Advance Clinical Practitioners on medical management and supporting, but not responsible for, supervision of the trainees (band 7 ACP posts appointed) as appropriate to their experience
When working in the community
* Support of Community Matrons and specialist nursing teams by means of advice, joint visits and education (in any appropriate/opportunistic form).
* Home visit assessments as appropriate.
* In response to COVID19 a rapid and successful shift was made by SHFT teams to wider use of telephone triage and zoom videoconferencing assessment and follow up of patients. It is planned that this will continue alongside face to face visits in the longer term.
* Attend, where appropriate, GP Virtual Ward meetings, which occur regularly in all practices in the locality and are attended by CNSs from the teams wherever possible.
When working in CAU
* Medical assessment of patients referred to the RAU. Both individually and alongside team colleagues.
* Provision of expert diagnostics and symptom control, disseminating principles and practice of Rehabilitative Geriatrics in the management of such patients
* Support of clinical teams in making difficult clinical and ethical decisions.
