Job summary

The West Hampshire Community Respiratory Service are looking for an enthusiastic and forward-thinking physiotherapist or nurse with a passion for respiratory to join our award winning and friendly multi-disciplinary team. The post holder will provide patient centered integrated care within pulmonary rehabilitation and home oxygen assessment and review, supported discharge and admission avoidance at various locations across West Hampshire. This role will be working predominantly in the Mid Hampshire area but occasionally covering in all other localities provided by the service.

The successful candidate will have a passion for respiratory, be highly motivated and able to motivate others. They will be able to evidence excellent communication with patients, families and professionals and be committed to excellence in the workplace. As a service we are committed to teaching and the development of our staff. All staff are expected to embrace the opportunities to learn and develop skills and knowledge to further their careers in respiratory care.

We are keen to hear from physiotherapists who do not fully meet the person specification as we may be able to offer a developmental opportunity to progress to a band 6.

A car driver is essential as the post will involve travelling across the region.

Main duties of the job

To work as an Autonomous Practitioner within the respiratory team, accepting referrals, providing comprehensive expert assessment and delivering care to individuals and groups of patients with respiratory conditions.

To provide a service within localities across West Hampshire covering pulmonary rehabilitation, oxygen prescription and management, admission avoidance and supported hospital discharge. The candidate will also be involved in delivering education to other professionals and staff across the locality. They will work closely with primary care, respiratory consultants, community teams and other specialist services.
