The Mosaic Primary Care Network has an exciting opportunity for you to join our team!

We are looking for a Registered Nurse to join our dedicated and highly professional team of Nurses working across the Basingstoke , Odiham and surrounding rural areas. You will play a pivotal role in keeping hospital admissions and re-admissions to a minimum as well as encompassing the dimensions of health service planning, advocacy and care planning for patients with a wide range of service needs.

Within this role, you will act as part of an ever-expanding multidisciplinary team, developing collaborative, needs based care plans, through assessment, planning, implementation and evaluation.

Additionally, you will be required to provide important educational and advisory service for patients, offering information on various aspects of health care to help support patients living with their condition. Within this role, you will be expected to provide nursing intervention to patients which include; wound management, catheter care, medicines management as well as palliative care.

You will need to be effective in communication skills and must be prepared to work flexibly and collaboratively to meet the needs of the service.

Main duties of the job

Working within Professional guidelines and accountability, the post-holder will have responsibility for the management of patients on a defined community caseload for a group of GP Practices (PCN) as part of the Integrated Care Team.

Work two days per week you will be released to work with the St Michael's Hospice team to enhance and develop your knowledge within the specialist palliative care field. Your time will split between the inpatient team and hospice at home to give insight and breadth to develop knowledge. You will take this new knowledge learnt at SMH and share with community colleagues. The role provides you with an opportunity to build and develop relationships with the specialist palliative care services and encourages the promotion of this to the wider Southern Health services.

The post holder will have responsibility for managing, co-ordinating and delegating work ensuring referrals are dealt with according to clinical need, holistic assessments and care plans are recorded in line with Trust and Professional record keeping standards and clinical care at all times meets acceptable standards of patient safety and quality outcomes

The post holder will be required to work with a high degree of autonomy and professional accountability. A primary aim of the role is to prevent unnecessary admission of patients to an acute hospital and support timely discharge into the community.
