The Team maintains our focus on providing quality Home Treatment both during supported discharge and as an alternative to Hospital admission.

You would be joining a Trust that has a Philosophy of Recovery based practise, and you will have the opportunity to contribute to the working within an established CRHT Team.

We are currently in the process of expanding our services and are developing the services in both the Basingstoke and Winchester localities with separate team bases for each service.

Our aim is to provide the kind of Service that we would expect for ourselves, or for someone that we love and care for. A service that promotes self-direction, independence and choice, that respects the individual, and provides the treatment and help that is needed as a priority.

As a Band 6 practitioner you will be taking a lead role in the team, supervising and supporting other team members, and will be expected to have the skills and knowledge that will facilitate effective care being delivered to service users in their own homes.

Main duties of the job

To be an active member of the multi-disciplinary team supporting the assessment, care planning and home treatment for our service users as either an alternative to hospital admission or in the transition phase when returning home from hospital.

To undertake ongoing assessment, treatment and evaluation for service users known to the service

To support the delivery of therapeutic interventions in the community setting during some of the more challenging times for the service users and their carers.

Support the transformation of the team to deliver local evidence based care.

The post-holder will develop and maintain professional supportive relationships with all members of staff with other professionals and agencies to enhance recovery and co-ordinate and monitor the delivery of person centred healthcare.
