Job summary

Seeking a mythical beast or two, Care of the Elderly/Frailty/Rehabilitation Consultant Geriatrician/s wanting to work primarily in community services. Southern Health NHS Foundation Trust, the largest community Physical and Mental Health community provider, is developing its frailty services utilising the sensible bits from Urgent Care Response and Virtual Ward national guidance.

We are seeking a Physician keen to sink roots into the Portsmouth and South East Hampshire area. Working across the South East Hampshire division of SHFT you will support and help mold our expanding team of consultants, nurse consultants, specialty doctors, Advanced Clinical Practitioners and community teams. Based primarily at Petersfield Community Hospital you will work alongside the Divisional Clinical Director/Community Geriatrician based at Gosport War Memorial Hospital, providing oversight to 4 wards, 2 Assessment Units, with linked Virtual Wards, and a single LDS Urgent Care response line.

Senior Specialty or Specialist doctors actively progressing CESR applications should consider talking to us. Interests in teaching, training or service development would be welcomed.Strong links to Portsmouth Hospitals University NHS Trust and funding would allow successful applicants to undertake specialist or acute sessions as part of a job planned portfolio, should they wish.

Petersfield station is 400 metres from the community hospital, with direct links to London.

Main duties of the job

Duties will be worked over 7-10 programmed activities. All job plans are reviewed in line with national guidance with commitment acknowledged for out of hours work

Job responsibilities

Wards

There are two rehabilitation wards at Gosport War Memorial Hospital (Ark Royal and Collingwood wards) and Petersfield Community Hospital (Rowan and Cedar wards), with a total of 84 beds. Usually, most patients on the wards have step-down rehabilitation needs. The wards take patients over the age of 18; though nearly all patients are older with problems related to frailty.

The wards offer step-up care to local patients, admitted via GPs or community teams. Linked to Virtual Wards and UCR we are developing this step-up programme of work, to offer local patients' better coordination of care with community and primary care, and as an alternative to acute admission.

Day to day medical cover of each community hospital site is provided by a combination of ward doctors and advanced clinical practitioners. We are keen to embed a mixed senior clinical workforce model that draws out the varying and complementary skill-sets of people from different professional backgrounds. Day-to-day clinical responsibility will be held by the Nurse Consultants, who carry clinical accountability for patient care at each site. The post holder/s can either split clinical responsibility with the Nurse Consultant or provide expert clinical input across the wards. The post holder will be expected to provide ongoing clinical supervision to the Nurse Consultant, who in turn will support the ward doctor, band 8a ACPs and trainee band 7 ACPs. Hospital board rounds and review of selected patients will provide the bulk of this supervision.

The proportion of time spent at each site will be established in discussion with consultant colleagues as part of job planning prior to commencing in post. A Service Level Agreement is already in place with PHU for Elderly Care Consultant provision into PCH; it is planned that this will continue and provides a relationship with PHU upon which to base discussions around any hospital sessions that applicants for these posts may want to explore.

Community Assessment Unit

There are two Community Assessment Units in South East Hampshire -- one at Gosport War Memorial Hospital and one at Petersfield Hospital. At present, these units provide rapid access multi-disciplinary falls assessments, as well as supporting patients with frailty related problems requiring comprehensive geriatric assessment.

The Community Assessment Units are now developing to provide a coordination hub for multidisciplinary assessment provided wherever this is most effective for our patients. With patients requiring follow up and closer monitoring being admitted to a Virtual Ward.

A Medical and Nurse Consultant model of oversight is being developed for each CAU and Virtual Ward, with UCR Matrons (1 for PCH and 1 for GWM, already in post) holding responsibility for the two areas.
